How to measure AI answer accuracy

Build a fact set, grade the answers, trace the source, fix it and retest.

The short answer

Measure AI answer accuracy by building a fact set of what must be right, grading each answer as accurate, outdated, incomplete, wrong or unofficial, tracing each error to its source, fixing it at the source and retesting.

Measure accuracy in five steps

  1. Build a fact set

List the facts that must be right: rates, hours, eligibility, locations, policies, names.

  2. Grade answers

For each prompt, mark the answer accurate, outdated, incomplete, wrong or unofficial.

  3. Trace the source

Find which page or site each error comes from.

  4. Fix at the source

Update your page, structured data or summary, or correct the third-party source.

  5. Retest

Run the prompts again and track the accuracy rate.

Error types and typical causes

Error typeTypical causeTypical fix
OutdatedAn old page, PDF or third-party listing is still cited.Update or retire the old source; add visible last-updated dates.
IncompleteThe answer exists only in a PDF or buried in a long page.Publish a clear page that answers the question first.
WrongConflicting sources, or a competitor or aggregator filling a gap.Publish the authoritative answer and correct third parties.
UnofficialAI relies on forums or reviews rather than your content.Make the official source easier to find, read and cite.
